Outreach, case management, and homeless prevention, and rapid rehousing services are available for individuals and families who are homeless or at imminent risk of homelessness.
- Case Management (PATH): Case management and therapy services who have a serious mental illness and reside in Pottawattamie, Harrison, or Mills counties in Iowa.
- Homelessness Prevention: Short-term financial assistance and case management services.
- Rapid Rehousing: Short- to medium-term financial assistance and case management services.
Eligibility: Services are provided in Pottawattamie County in Iowa.
