Heartland Family Service

Community Support

Heartland Family Service Community Support Services benefit clients who are eligible for Iowa Medicaid and have a primary behavioral health diagnosis aside from Substance Abuse Disorder. Initial intake with a client consists of describing the benefits and community support guidelines expected of the client. Staff will complete a crisis plan with clients and ask specific questions to best meet the client’s needs. Each client is unique and has different needs. After 30 days of being enrolled in the service, a case plan is completed. Staff assist clients in reaching their goals by using specific interventions. Case plans are updated every three months.
