PATH

The purpose of PATH (Projects for Assistance in Transition from Homelessness) Case Management is to provide services to individuals who are homeless and have a serious mental health condition or co-occurring substance use disorder. The goal is that case management and supportive services provided will enable those who are homeless or at risk of becoming homeless to obtain and maintain appropriate and stable housing.
