Bridges
Heartland Family Service Bridges offers two services, Transitional Living and Crisis Care.
Bridges Crisis Care is a residential crisis stabilization service that offers a safe, secure, and trauma-informed environment for individuals experiencing a mental health crisis to receive access to daily therapy, medication management, and intensive crisis intervention services. The goal of the voluntary 3-to-5-day service is to assist clients in understanding their crisis, stabilizing their symptoms, and connecting to community-based resources promoting individual stability as they return to the community where they live. Bridges Crisis Care referrals are accepted 24-hours, 7-days a week via phone call to Bridges at (712) 325-5650. A live screening is conducted in real time, and individuals are provided immediate admission if approved.
Bridges Transitional Living provides short-term housing enriched with supportive services designed to assist individuals with a mental health or dual-diagnosed conditions while they transition to stability in the community. The goal of the program is to connect clients to community-based mental health or substance use treatment services and develop a plan to secure benefits, income, and permanent housing to promote individual recovery and stability in the community.

Eligibility
Individuals age 18 and older with a verifiable diagnosed mental health or co-occurring condition that reside in the Southwest Iowa area. Participants must meet all Southwest Iowa Mental Health and Disability Services Region eligibility requirements.

Fees
Residents pay 30% of their gross income toward facility room rent.
