We care for clients from the very beginning of their wellness journey, guiding them through what can be a complex system of social services. Our interdisciplinary team ensures seamless support across programs and agencies.
A Certified Community Behavioral Health Clinic (CCBHC) model of care is also designed to expand access to services including crisis care, mental health and substance use treatment and physical health pathways.
HOw DOES IT WORK?
Clients enroll directly through Heartland Family Service or
by referral. Clients are guided through the programs that
meet their needs, ensuring that they are connected to both
internal and external resources.
These Heartland Family Service programs are available through the CCBHC continuum of care:
- Patient-centered treatment planning and individualized case management
- 24/7, community-based crisis services
- Outpatient mental health, substance use, and medication management services
- Peer and family support
- Assertive Community Treatment (ACT) for adults who need more support than traditional outpatient services
Services are available to all individuals, including veterans
and military members.
