Permanent Supportive Housing

The following programs provide long-term financial assistance and supportive services for individuals and families who are experiencing long-term homelessness with one or more disabilities, such as mental health, substance abuse, or a physical disability. To be eligible, individuals or families must be chronically homeless (living in a shelter or on the streets for a long period of time) and have a verified disability. Referrals are accepted through the community’s coordinated entry system.

Heartland Housing Solutions
To be eligible, Individuals and families must be chronically homeless (living in a shelter or on the streets for a long period of time) and have a verified disability. Services are provided in Pottawattamie County in Iowa or Douglas and Sarpy counties in Nebraska. Some openings are reserved for former wards of the state ages 18-24.

Heartland Homes/Transitions
Provides facility-based, long-term housing to individuals and families in Council Bluffs, Iowa who are literally homeless and have a disability. To be eligible, individuals and families must be chronically homeless (living in a shelter or on the streets for a long period of time), have a verified disability and income must be at or below 50% of Area Median Income (AMI). Services are provided in Pottawattamie county, Iowa.
