Heartland Housing Connections

The purpose of our Rapid Rehousing programs is to provide assistance to rapidly re-house persons who are homeless. The programs provide temporary (one to 24 months) financial assistance and support services.

Heartland Housing Connections targets those experiencing long-term homelessness with high-service needs. This program provides intensive case management services to help support individuals in sustaining long-term housing.

Eligibility: Individuals must be literally homeless and living in Pottawattamie County in Iowa or Douglas and Sarpy counties in Nebraska.
